We do not support adult perpetrators of violent and/or sexual offences, in this event we will signpost to more appropriate services.

SRASAC is part of the South Yorkshire ISVA Service which delivers the ISVA service across South Yorkshire together with specialist sexual violence services in Rotherham, Barnsley and Doncaster.  All referrals are received centrally for South Yorkshire and then passed on to the relevant local service.

You can self refer into the ISVA service or ask for a referral to the service from the Police, NHS or any other agency.

1. Referral Form:

The first step to access ISVA Support at SRASAC is to complete our Referral Form which asks for personal information about you and how best to contact you. It would help us to help you, if you list as many ways as possible that we can contact you. If you find it difficult to complete this form, please ask someone you can trust to help you, this may be a support worker or GP, friend, relative or partner.

Some of the questions asked on the form may not make sense if you are referring yourself – please ignore any questions that don’t make sense to you eg referrer details and incident details if this is too upsetting.   Please call our Information and Support Line on 07517 100 757 or the office on 0114 241 2766 if you need help in completing it.

The data provided on the referral form is used to identify clients and assist the ISVA worker when contacting agencies as part of their advocacy role. This data on this form is treated as both 'personal' and 'sensitive' and subject to the policies of the local ISVA service provider with respect to confidentiality and data protection.

2. Contacting You:

When we receive your completed form at SRASAC, we will try and call you within 48 hours of receiving the referral to explain the ISVA service to you and answer any questions you may have.  You will also be given the names and contact details of the ISVA Team if you wish to contact us at any time.

3. Further Information Needed:

We will then send you an email with a link to a secure Risk and Needs Assessment form which asks for more information from you, this helps us ensure that we are able to offer the right service for you.  We can send this letter and form to you by post of preferred.

When we receive the completed form back, you will receive confirmation that we are now supporting you as an ISVA client at SRASAC.  We will give you the names and contact details of the ISVA Team so you can contact us at any time.  We will arrange regular check ins with you and you will be allocated a named ISVA as your case progresses.

4. Confidential Service:

All information you provide is confidential; this means it is kept safe and secure (see full data protection statement below) and will not be passed on to anyone or any other organisation, outside of SRASAC.  If you have any questions, or need help to complete this form please call our Information and Support Line on 07517 100 757 or call the office on 0114 241 2766.

5. Data Protection

Data Protection Statement from SRASAC: This data protection statement sets out how we’ll use your personal data. SRASAC will collect your personal data (which includes sensitive personal data) and process your personal data for the purposes of providing you with support.  The personal data collected on this form will be kept secure and confidential within SRASAC.  Your personal data will only be used as necessary for our legitimate interests in order to provide you with support and monitoring within SRASAC. 

This information will never be disclosed to any external sources without your express written consent or unless we have significant safeguarding concerns related to immediate risk to a child or vulnerable person (See SRASAC safeguarding policies in relation to this). 

SRASAC does share anonymised and unidentifiable information with funders in support of our work. By ticking the box below you are giving your consent for Sheffield Rape and Sexual Abuse Centre to hold the information given on this form about you.  Where Consent has been given you are free at any time to change your mind and withdraw your consent by contacting us at [email protected] .  Where consent has been withdrawn, SRASAC will delete any personal information we have stored.  SRASAC will retain your personal data only for as long as is necessary for the provision of support.   This statement is in accordance with The General Data Protection Regulation (GDPR) (EU) 2016/679
